How do I use this plugin?

Hi, I've downloaded and installed this plugin but am baffled as to what to do next. There appear to be no settings in the dashboard. How do I get the feedback form to appear on my forum. Do I need to set up a route and edit a template?
Forgive me, I'm a first-time Vanilla2 user and I could use some instruction with this plugin. Many thanks.
