My file upload is failing? Any help?

Hi I am using FileUpload Plug-in. It is letting me to select however it is not getting uploaded. PHP image upload is working for avatar/banner etc. So php is configured for taking uploads. I don know why attachment fails. Any one came across such weird issue so far? Any help appreciated.
