File upload - image not showing

Hi
I have embedded vanilla into my site. In trying to attach a file, I can see the image once I have 'chosen file' but on clicking 'insert image' the following is inserted:
so obviously no image appears.

I see I'm not alone with this problem: [FileUpload] [FileUpload Addon] Selected 'Insert Image' after upload, no feedback to user?
Just hope there's a solution soon.
