another "automatic email notification" question

so I'm running 2.0.18 and I see the " Notify me when people comment on my discussions." setting in preferences. but I really would like users to control their notification preferences on a "per discussion basis".

(and does the above setting include discussions started by others but where I joined?)

I've seen the request be made and saw some plugin (and ideas for plugins) but has anyone made one that adds a checkbox or dropdown to the comment box on each discussion so that users can decide which discussions to follow??

Bookmarking a discussion subscribes you to notifications... I dunno it seems kind of redundant to add another control to have them subscribe to it.
