Creating a form

hi im pretty new to vanilla and still exploring, can someone teach me how to create a form in a plugin?

ex.
process:
1.what's your name ?
2. captcha
3. submit.

and will be sent to the database? i've tried loading up example plugin but i cant seem to figure it out yet.
