Implenting Captcha For Creating Discussions

I am having a large problem, now that my site has grown, with spam.
Regardless of how many IP's and e-mails I block, they keep re-appearing. I've implementing numerous ineffective anti-spam plugins as well as requiring e-mail verification.
My only other idea is to force them to fill out a reCAPTCHA to make a post. However, the thing is: They have to fill out a captcha to register in the first place. So however they bypassed it before, they can do it again.
Maybe I should be asking: Is there a way to maybe ask a special question on registration to verify they are human or change it to a more effective captcha?

Is it approved members spamming or applicants.
do you use botstop. how do you approve applicants. some of these questions and how you do things could possibly be imporved. putting a captch on adding discussions or comments would stop me from using a forum (just my personal opinion).
how do you block ips? deny them via .htaaccess then they won't even see your site.
