Integrating a custom 'plug-in' into Button Bar

Afaik this was successfully integrated into YaBB, vBulletin and phpFox (as a plugin for TinyMCE).

YaBB_Integration_Guide.txt is included in this archive - if you care to take a look.

Basically I want this same thing done but in Vanilla:

My question is how can i integrate this into phpfox forum?
DO i have to modify the WYSWYG editor? Or do i have to modify the core files of the forum?
Can i do this as a plugin? Or do i have to create a new module?

I'd prefer Button Bar, but any editor would be fine. Thoughts?
