How to create 2 columns for comments based on field?

I would like to create a new field on the comment field (drop down with “yes” or “no”) that displays next to the persons comments. I also want two columns on a discussion (yes or no) and based on the comment field, it should place the comment into the correct column. Thanks in advance.
