Admin starting new discussion or commenting is no longer sending notifications in 3.3

I have two problems going on that hopefully someone can put some insight into:
The admin account is no longer notifying everyone when a new discussion is opened or when the admin has commented on a discussion.
The member accounts are working fine, but all the notifications that get emailed have failed to have the title of the site in the "from" box, and arrive only as [ ].
As far as I can tell, all notifications are set up properly, and, it was working fine, and now it's broken. Plug ins do not seem to be the problem, it remains in any plug in configuration.
