Extended Application Form doesn't save the fields to the database (SOLVED)

I just installed ExtendedApplicationForm 1.0 and discovered that though it asks for some extra information (First and Last name), when the applicant is granted membership, those fields ARE NOT COPIED INTO THE DATABASE. Any ideas on how to modify it so that it does? Thanks

    I believe the extension is supposed to add the fields for you. Try disabling and enabling the extension again - that is when it is supposed to create the fields.

    That's right. There was a problem in my local test forum, I solved it and everything worked just fine. I just uploaded the extension to my forum. Thanks Mark!
