How do i make a separate page for the settings of a certain extension, like in the page manager?

The extension.php file exists for this reason.

Or do you mean where you input the settings? You can find it in the add-on where it begins:

if(($Context->SelfUrl == 'settings.php') && $Context->Session->User->Permission('PERMISSION_CHANGE_APPLICATION_SETTINGS'))

For a couple of simple settings in the textile add-on update I looked at another add-on (account pictures I think) and 'borrowed' code.
