Just upgraded to 1.1.4 and it works but it does not have an extensions option in settings (I don't remember it ever having one). You need to install the tables using the link provided in the extensions text, then change the array at the beginning of default.php as described. The new input field(s) should then be added to the sign-on field and the user profile. Check the readme that comes with the extension.
