Unable to access any module post upgrade to 2.5.1

After completing upgrade from 2.3.1, when I click any of the modules (APPEARANCE-> Branding, Layout, Themes, Avatars, Emails and a lot more) it redirects to the same page.(Please find attached screenshot) Also when I visit http://{Path}/{To}/{Vanilla}/settings/tagging, the script throws error "Something has gone wrong." All this happens only when I upgrade from 2.3.1 version, on fresh installation it is working fine.
Do I need to change anything during upgrade process other than what is written in README.md file ? I have followed the steps written in README.md for upgrading. Or there is any modifiacations in .htaccess file is required ?

I had somewhat the same problem.
You have to delete /applications/vanilla/controllers/class.settingscontroller.php if you're upgrading from <2.4.
