    Update: I'm told a major performance issue *does* exist with that patch (the "or" condition kills query speed to an unacceptable degree on large forums) so going to keep trying until an acceptable solution is found. :)

    What file would I need to edit to just remove the "Dismiss" option?

    You can disable it in conf/config.php by adding this:
    $Configuration['Vanilla']['Discussions']['Dismiss'] = FALSE;
