Vanilla 2.2 release candidate (RC1)

I expect a release date in the range of Nov 2-4. Barring new critical bug reports, I do not expect further work except security updates prior to then. Please test this release thoroughly and report issues on GitHub promptly.

The 2.3 release cycle will likely begin before the end of the year.

Changes since the beta:

• Reverted PATH_INFO / htaccess changes from 2.1 until further compatibility work can be done.
• Multiple security fixes.
• @R_J fixed mentions in brackets & advanced notifications sending wrong message when a mention was included
• Minor coding standards fixes.
